In whatever way the referral is received, the next step is to complete the Residential Application, and tour the facility.
Step Two: Tour / Assessment
An assessment of an individual's needs must be completed prior to admission. This can be completed at home or in the hospital. Input from the individual's caregivers, family members and physicians are all necessary to meet that individual's needs. The result of an assessment will be acceptance or a "re-referral" to another level of care. It is important that the individual, the individual’s caregivers, family and decision-makers tour the facility to assist in the assessment process.
Step Three: Admission
Once evaluated and accepted, the individual or their responsible party will complete necessary paperwork and establish plan for admission. Documents needed for admission include History and Physical (within 1 year) and a signed list of medications from MD.
Day of Admission
The Day of Admission should be a calm experience. Too much information can be overwhelming. Upon arrival, a member of the staff will meet you at the entrance and assist the individual into the building. We will arrange to transition the individual into an activity or meal. The person accompanying the individual may then speak with the staff, relaying any pertinent medication information or care issues. Staff will provide assistance on ending the visit and be available to assist in anyway.
